This form must be completed by a qualified Caseworker before matching a Mentor if any of the below are true:

a. a Community-Based 1:1 Mentor is being rematched

b. it has been 5+ years since the Mentor was last screened (regardless of program type)

c. it has been 1+ year since the Mentor was active (regardless of program type)

The form should not be completed by the Mentor’s primary Caseworker to ensure the review is objective.
