PRISM – Pride, Respect, Identity, Safety, Mentoring

PRISM Community Needs Survey

The purpose of this survey is to collect information on current gaps in service for the 2SLGBTQIA+ community within York and Peel Regions and Bradford West Gwillimbury. The information collected will inform the development of the PRISM program at BBBSPY as well as it’s future programming. The form will also collect personal information for those who want to become a mentee, mentor or advisory panel member. The form is directed towards Peel Region, York Region, and Bradford West Gwillimbury service workers and volunteers who work and/or volunteer with the 2SLGBTQIA+ community, families members of 2SLGBTQIA+ youth, and 2SLGBTQIA+ youth and adults.

SHARE YOUR VOICE AND BE PART OF THE IMPACT

PRISM stands for Pride, Respect, Identity, Safety, and Mentoring. BBBS of Peel York is developing a PRISM program that best meets the needs of the 2SLGBTQIA+ community. To do that, we need your input! If you are an 2SLGBTQIA+ young person or adult, family member of a 2SLGBTQIA+ young person, or an organization that serves the 2SLGBTQIA+ community directly or indirectly, we want to hear from you! Your answers will better inform the development of our PRISM program to ensure we are filling the gaps in services currently offered within York and Peel Regions and Bradford West Gwillimbury. As compensation for your time, we will be giving out five e-gift cards of $10 value and will be drawing for a winner after every 10 entries. The draw is optional. To be included in the draw, there will be a space to enter your preferred name and email address at the end of this survey. BECOME AN ADVISORY PANEL MEMBER

As part of the development and ongoing success of the PRISM program, BBBSPY will build an Advisory Panel. This panel will meet on an ongoing basis that is established and agreed upon together by the Advisory Members and the PRISM Program Coordinator. The Advisory Panel will have direct involvement with the creation of the PRISM program, the assessment of the program, marketing of the program, program materials and more. We are hoping to have two panels, the PRISM Youth Advisory Panel (for those under 18 years old) and the PRISM Advisory Panel (for those over 18 years old). If you are interested in joining the Advisory Panel or learning more about it, please fill out your information below and our PRISM Program Coordinator will be in contact with you.
