Serious Situation Response Form – BBBSPY Local Practice
This form is to be completed by Agency Staff to record incidents, situations, disclosures, and/or discoveries that impact the well-being of members of the BBBSPY community (i.e. Mentees, Mentors, volunteers, families, staff, placement students, etc.) that may or may not be related to their involvement with BBBSPY.
The incident or situation must still be documented in SDP as a case note. This form should NOT be used in the event of an allegation of abuse related to BBBSPY participation. In those cases, BBBSC Appendix C – Confidential Reporting Form – Allegation of Abuse must be completed. Examples of Serious Situations include: – Disclosure of abuse unrelated to program participation, – Self-harming behaviour, – Mental health crisis, – Threat to safety, – Extreme food or housing insecurity, – Personal or family emergency , – Police or child welfare involvement, – Conflict between program participants, – Injury or incident at BBBSPY program or event, etc. When in doubt, please contact your Supervisor.Share:
