Volunteer Transition Re-Assessment: Self-Report Form

The Volunteer Transition Re-Assessment is part of a nationally required process that must take place before any Volunteer Mentor can be rematched with a new Mentee or group of Mentees.

For Mentors in Group programs, this process is required each summer before a new school year begins, and/or if it has been 3+ months since you were last involved in a group mentoring program. For Mentors in 1:1 programs, this process is required before any new match can be made. Please answer all questions honestly and thoroughly. A member of our team may follow up to confirm or clarify your responses.
